Hi I’m Liz, I’m a Care Co-ordinator and I work out of the Rushall Medical Centre, Collingwood Family Practice and Blackwood Health Centre.

  • Provides holistic care to all Patients

  • Provides expert support to Patients who are directly or indirectly affected by cancer

What things do our Care Coordinators support with?….

  • Supports Patients who have been directly or indirectly affected by a bereavement

  • Works with patients who are living with frailty or long term conditions

  • Facilitates and run specialised events in Walsall East 2 PCN Practices

  • Helps Patients with Physical and Mental Health Conditions

  • Works closely with the Primary Care Network Team ARRS Role and GP’s

  • Uses close relationships with professionals through health, social, voluntary and community sectors to provide the right support from the right person at the right time
