Clinical Pharmacists

Clinical Pharmacists will complete your Structured Medication reviews. These are chats either face to face or on the telephone, reviewing your medication. These reviews help to make sure you are taking your medication safely and that you know your medication.

First Contact Physiotherapist (FCP’s)

If you have back or joint pain, e.g. arthritis, you can be booked to see a First Contact Physiotherapist (FCP) rather than a doctor or going to the hospital. This means you can be seen more quickly, receive expert help in managing your condition better and recover faster.

Clinical Staff

Paramedics

Paramedics can help the doctors and nurses by seeing patients for acute problems. Paramedics also carry out home visits. Paramedics can work with patients who have a long-term illnesses, particularly where the patient is housebound and therefore cannot attend the practice.

Pharmacist Technicians

Pharmacy Technician – help our Clinical Pharmacists. They can deal with prescription questions and can work with you to get your medication right for you. They cannot make clinical decisions but work to make sure you are getting the best from your medicines.

Social Prescribers

Social prescribers help doctors and nurses to take a full approach to your health and wellbeing. They can join you to community groups and services to help you.

Non-Clinical Staff

Care Coordinators

Our Care Coordinators aid our Social Prescribers. They order and invite you to events ran by our Team. They also reach out to recently bereaved patients and patients who have been affected by a cancer diagnosis and will signpost to support.

Health and Wellbeing Coaches

Can work with you to improve your health through lifestyle changes. This will involve 1-1 sessions. Health coaching is not about the coaches telling you what to do, it is about the coaches listening to you and guiding you to better health.
